FELLOW LOGO AGREEMENT

This symbol is a registered trademark and may be used only by active Fellows of the American College of Osteopathic Surgeons (ACOS).

The symbol is only to be used for the limited purpose of indicating Fellowship in the ACOS. All uses of the symbol must include the Fellow’s name, a statement that the individual surgeon is a Fellow of the American College of Osteopathic Surgeons, and the ® designation. The full College name, “American College of Osteopathic Surgeons,” must be used for that purpose. A group practice name may not be used in place of the Fellow’s name.

The symbol may be used on practice stationary, brochures related to the Fellow’s practice, practice forms, business cards, websites, advertising, personalized labels or stickers, plaques, office buildings, office furniture, and gifts for patients. The symbol may not be modified in any manner. No additional material or text may be placed immediately adjacent to or inside the symbol, except for the ® designation. The ACOS retains the right to review and approve any Fellow’s use of the symbol.

It is the responsibility of the individual Fellow to ensure that the use of the symbol conforms with the AOA Code of Ethics and AOA and ACOS interpretations. The user of this symbol may not create any impression that the symbol is evidence of /or an indication of the user’s compliance with the College’s Bylaws and AOA Code of Ethics and interpretations.

The following statement is recommended for all uses of the symbol where possible:

ACOS Fellows are committed to assuring excellence in osteopathic surgical care through mentorship, education, advocacy, leadership development, and the fostering of professional and personal relationships.

The user of this symbol for any purpose shall comply fully with all regulations and guidelines of state and national regulatory authorities, state medical boards, the American Osteopathic Association and the Federal Trade Commission.

The ACOS is the owner of the symbol, the Fellow by executing this license is granted a limited license to use the symbol for the specific purpose of indicating their Fellowship in the ACOS, and the ACOS may withdraw permission to use the symbol at any time for any reason. Upon notification of withdrawal of permission, the Fellow will immediately cease use of the symbol.

By use of the Fellow logo, I understand and agree to be bound by the terms and conditions of use as set forth in this licensing agreement.
